Hi Nishit the user input form pop up not quite correct as it neeeds to be set into the mid column, also the fields that reqired mentioned in the video i can not see any reason for us to contact site admin to check it as they all showerd to Andrey which fields they want and which not. In you draft it looks copy from inteligent i do not think it is a good idea to show site admin this druft as this is not what they expect for sure.
The fields must be in side of the mid column and should be very compact and modern,
would you please chekc the video what fields reqired and fit it inside of the mid window.
